Letter from the Presidents
We are happy to announce the 2023 plans and benefits offered through the Jacksonville Police Officers and Fire Fighters Health Insurance Trust. The Trust had an opportunity to market our benefits in order to find the best plan designs and carriers out there. We considered cost, network access, plan design, value adds, and overall service when making our selections. And we found the best options for both the members and the Trust was to keep our current carriers – Florida Blue, Delta Dental, and EyeMed. And we are adding Aflac as our new supplemental plan partner for 2023.
As you may be aware, health care costs have been increasing substantially over the years. Specifically, we have seen increased costs in two of our medical plans – the Blue Options PPO 5782 and the BlueCare HMO 48 - while the UF Direct Health and BlueCare HMO 65 have remained stable. Though we have been able to maintain consistent levels of plan costs in the past, we have no other option but to share some of these costs with members. As a result, the costs of the Blue Options PPO 5782 and the BlueCare HMO 48 will increase for 2023. The rates for the UF Direct Health and Blue Care 65 plan remain the same. We were able to find new products and savings on the dental, vision, and voluntary plans. You will notice a combination of lower premiums and enhanced benefits throughout the plans.
We are committed to providing you with benefits to meet the needs of both you and your family. This means a commitment to quality, value, and benefit options. Enrollment sessions will be held in October to all members and you can find additional materials on our website. You are encouraged to visit our sessions and carefully review the enrollment options to ensure your health insurance selection meets your needs and budget.
Medical
You have the choice of four medical and prescription drug plans through Florida Blue and Express Scripts.
Product Highlights:
- Plan rates are increasing slightly for the Blue Options PPO 5782 and Blue Care HMO 48. The UF Direct Health and Blue Care HMO 65 rates remain the same. The copays, coinsurance, deductibles, and annual maximums remain the same for the PPO and HMO plans.
- The UF Direct Health Plan will now be administered by Florida Blue. You will have access to the same providers and facilities as part of the exclusive UF Health network. And you will have the same low copays and cost sharing for services. The prescription drug benefits are now offered through Express Scripts National Preferred formulary. The UF Health plan will now include $0 Teladoc visits and $0 copay for Tier 1 Generic prescriptions. All UF Direct Health enrollees will get new medical and prescription drug ID cards with new member IDs.

Dental
You have the choice of four dental plans offered through Delta Dental. The new plans include benefit enhancements and lower premiums.
Product Highlights:
- All dental plans will offer one additional cleaning, totaling three cleanings per calendar year.
- The Gold and Platinum plans include enhanced coverage for Endodontics, Periodontics, and Orthodontia. Members will now pay the plan deductible and 20% coinsurance for endodontic and periodontic services. Orthodontia services are available with 25% coinsurance up to the plan’s lifetime maximum of $2,500 for the Gold or $5,000 for the Platinum.
- There are no waiting periods for services in all 4 dental plans.

Vision
You have the choice of two vision plans offered through EyeMed.
Product Highlights:
- Both plans include a new network option called Eye360. Eye360 provides enhanced benefits of a $0 exam copay and an additional $50 towards frame allowance when visiting one of 4,000 PLUS Providers nationwide.
- 39% of our members are already using a PLUS Provider which means there are additional savings for you. Eye30 can also be combined with special offers to allow a bigger savings to our members.
- You can find PLUS providers by visiting www.eyemed.com, searching for local providers in the Insight Network, and look for profiles showing the PLUS Provider icon.

Voluntary Products
Supplemental products will now be offered through Aflac.
Product Highlights:
- Aflac brings on a variety of new enhancement to each plan and offer lower rates to our members. Active members can elect Critical Illness, Accident, and Hospital Indemnity plans.
- Retiree members who are actively working in other job role can now elect the Aflac accident policy.*
- An Aflac representative will be available at each of our open enrollment sessions for members who are enrolled in an individual plan now and want to compare to the new JPOFFHIT products.

Accident
Accidents happen. Financial hardship doesn't have to. Aflac pays you cash benefits in the event of a covered injury, so you can focus on recovery instead of your finances.
Critical Illness

Hospital Indemnity

New Wellness Product
Active members will have access to both in-person and virtual on-demand fitness programming at no cost through the Peerfit platform and mobile app.
Product Highlights:
- Peerfit offers a flexible fitness solution that is easy to use and can help you lead a more active and healthier lifestyle.
- Members receive 15 credits to use towards individual fitness sessions or monthly gym memberships, with a network of over 11,600 fitness studios and gyms available all over the country.
- The virtual on-demand solution encompasses a variety of fitness categories including HIIT, cardio, yoga, strength training, meditation, toning, dance, and more.
- Members can use credits towards additional resources such as nutritional recipes and weekly meal plans.
